The Be Well: Youth Mental Health Service, is for young people (14 to 25yrs) across Limerick City & County who are in need of support for mental health & well-being.

Looking for a Referral form for the Be Well Youth Mental Health Project?

Referral forms can be accessed below. Where possible, we would appreciate if forms could be submitted via email.

When you form is submitted, an assigned team member will contact you to arrange a suitable time to meet the young person (or young person & guardian when under 18).  Support in completing referral form is available.

The Be Well Youth Mental Health Project provides:

  • Counselling
  • Support with a youth mental health worker (rural & city)
  • Parental support and advice
  • Supporting youth groups in promotion of positive mental health
  • Youth Mental Health Youth Advisory Panels (based in the city & county)

What sort of issues can we help young people with?

  • Alcohol & drug misuse
  • Support with emotions
  • Grief & loss
  • Dealing with your sexuality
  • Relationships
  • School
  • We can help a young person develop new coping skills in a supportive and encouraging space.

How it Works

You will need to complete a referral form which is available by clicking on the appropriate link below.  Due to the current situation with Covid-19, we would encourage all referrals to be submitted online due to limited access to our youth spaces.  All agency referrals are to be submitted online.

Please note that as part of the registration and consent process an Individual Data Consent Form must be signed and returned by over 18 self-referrals and parent/guardian referrals of under 18s.  This form is included under the appropriate link below.  This is to ensure LYS compliance with EU General Data Protection Legislation (GDPR), 2016.
